This form is for requesting refills of medication that has already been prescribed for your pet; new conditions will require an examination at the practice.

If your pet is due more of their usual medication, please order a minimum of 2 working days before collection, to allow time for medications to be ordered, dispensed and checked.

Please be aware that by law:

• A veterinary surgeon is only able to prescribe veterinary medicinal products following a clinical assessment of an animal under their care.

• Pets on long-term medications will require a prescription check including a physical assessment with a veterinary surgeon every 6 months as a minimum.

• Some medications require a physical assessment with a vet every time they are prescribed, such as antibiotics and controlled drugs.

 

The Small Animal written prescription fee is £20 inc VAT.  You may obtain veterinary medicinal products from us OR ask for a written prescription and obtain these medicines from another veterinary surgeon or pharmacy.
